Good to Know Beforehand
Make sure you have all the necessary information and photos on hand before filling out the form. This includes tire details, vehicle information, invoice details, and clear photos of the tire, the serial number, the DOT number, the tread depth, and the issue. This will allow you to complete the form in one go.
 

Step 1: Open the claim form

Click on “My Warranties” and, under “My Warranty Requests”, select “File a Claim”.

Step 2: Select the correct type of claim

Select the category for which you’d like to file a claim: truck tires, truck rims, agricultural tires, or earthmoving tires.

Step 3: Enter your information

Verify your contact information and enter the pickup address for the tire.

Step 4: Enter the tire and vehicle information

Fill in the requested information about the tire, the issue, and the vehicle as completely as possible. Fields marked with an asterisk (*) are required.

Step 5: Upload clear photos

Attach the requested photos, such as photos of the tire, the serial number, DOT number, tread depth, and the issue.

Step 6: Review and submit

Review all details and submit the claim. We will then process your request.
